General Description
The Triage Nurse plays a pivotal role in delivering safe, compassionate, patient-centered care remotely. This position collaborates closely with medical providers, including physicians, nurse practitioners, and physician assistants, as well as clinical and administrative staff to ensure high-quality, coordinated care. The Triage Nurse is responsible for assessing patient needs via telephone and patient portal messaging, providing clinical guidance, and directing patients to the appropriate level of care. This role promotes a culturally sensitive, inclusive, and respectful environment that prioritizes safety, continuity of care, and patient engagement.

Responsibilities

Specific Responsibilities of the Position
  • Conduct comprehensive assessments of patient symptoms through telephonic and electronic communications
  • Utilize established clinical protocols to provide safe and effective medical advice
  • Determine the urgency of patient conditions and recommend appropriate care pathways, including emergency services, primary care, or self-care
  • Document all patient interactions accurately in the electronic health record (EHR) system
  • Collaborate with healthcare providers to ensure continuity and coordination of care
  • Educate patients on managing their health conditions and provide resources for support
  • Participate in ongoing training and professional development to stay current with best nursing practices
  • Actively participate in the organization’s performance improvement initiatives
  • Perform other duties as assigned in support of the organization's mission and care delivery goals
Required Knowledge, Skills, and Abilities
  • Demonstrates clinical knowledge of a wide range of acute and chronic health conditions and provides compassionate care across the lifespan, including pediatric, adult, and geriatric populations
  • Communicates clearly and effectively with patients, families, and interdisciplinary team members, both verbally and in writing
  • Works independently while contributing to a collaborative team-based environment
  • Develops and delivers health education appropriate to patients’ varying levels of literacy and preferred learning styles
  • Applies knowledge of chronic disease management, preventive care, and population health in a community-based primary care environment
  • Demonstrates age-appropriate care and clinical judgment for patients of all age groups
  • Uses office equipment, technology, and software to perform clinical, clerical, and administrative duties efficiently
  • Demonstrates strong critical thinking, time management, and relationship-building skills in a fast-paced, work environment
  • Organizes and prioritizes multiple tasks and projects with accuracy and attention to detail
  • Performs accurate calculations for medication dosages, documentation, reporting, and data collection
  • Assesses patient calls/messages, identifies key issues, and recommends appropriate actions to support safe and effective care
  • Delegates tasks in accordance with licensure requirements, staff competency, and scope of practice
  • Demonstrates flexibility, initiative, and the ability to learn and adapt to new processes with minimal supervision

Skills & Qualifications

Education and Experience
  • Degree in Nursing from an accredited school of professional nursing required
  • Minimum of two (2) years of experience performing registered nurse duties in a clinical or ambulatory care setting preferred
  • Experience in community health, primary care, or public health settings strongly preferred
  • Bilingual skills preferred
Required Licensure and Certification
  • Must hold and maintain an active, unencumbered license as a Registered Nurse (RN) issued by the Texas Board of Nursing or an active, unencumbered RN license issued by a participating Nurse Licensure Compact (NLC) state board of nursing
  • Current Basic Life Support (BLS) certification required; must be maintained throughout employment
