The Behavioral Health Care Coordinator plays a vital role in the delivery of integrated behavioral health services by serving as a central point of coordination for patient care. This position facilitates seamless communication between patients, behavioral health providers, and medical teams, ensuring timely access to services and continuity of care. The coordinator supports patients in navigating the behavioral health system, reducing barriers to treatment, and promoting positive health outcomes through proactive care planning, follow-up, and resource connection.
The coordinator is responsible for managing and coordinating all behavioral health referrals by conducting brief needs assessments and scheduling patients with the most appropriate program, counselor, or psychiatric provider. This role includes providing timely and accurate community mental health referrals when needed and being prepared to respond to behavioral health crises in a calm and effective manner. Additional responsibilities include completing insurance verifications, and offering patients clear, upfront estimates of service costs prior to their initial appointments.

Responsibilities
Specific Responsibilities of the Job:
- Coordinate behavioral health care services, including scheduling appointments, follow-ups, and referrals.
- Act as liaison between patients, behavioral health providers, and primary care teams.
- Support care plans adherence through outreach and check-ins.
- Provide resources to patients about behavioral health services and treatment options.
- Identify barriers to care and assist patients in accessing community resources and support services.
- Work collaboratively with an interdisciplinary team to support integrated care models.
- Provide brief assessments to determine the most appropriate behavioral health program or service depending on each patient’s individual needs.
- Verify benefits, provide patients with options for financial assistance, and estimated cost for treatment prior to the patient’s first appointment.
- Monitor patient attendance and engagement with scheduled services, and conduct patient satisfaction surveys to support ongoing program evaluation and quality improvement efforts.
