The Community Health Worker (CHW) – Women’s Health Navigator plays a key role in expanding access to HIV and STI prevention for women served by Prism Health North Texas (PHNTX). This position focuses on culturally responsive education, trust-building, and navigation to prevention services by embedding prevention into routine women’s healthcare and trusted community settings.
The CHW is a frontline navigator and connector, not a clinical provider or marketing role. This position works closely with Women’s Health providers, HIV prevention teams, and community partners to normalize prevention, reduce stigma, and support linkage to care.

Essential Duties and Responsibilities

Patient & Community Engagement
  • Provide culturally responsive, trauma-informed education on HIV and STI prevention
  • Support HIV/STI screening workflows in coordination with clinical staff
  • Conduct outreach and education in trusted community spaces, including women’s groups, faith-based organizations, colleges, shelters, and community events
  • Build trust-based relationships with women and community partners to encourage ongoing engagement in care
Navigation & Care Coordination
  • Provide navigation support to PrEP and other HIV prevention services, including education, scheduling support, and follow-up
  • Facilitate warm handoffs between patients and clinical or prevention teams
  • Support continuity of care and reduce loss to follow-up through reminder touchpoints and patient check-ins
  • Collaborate with HIV Prevention and PrEP teams to ensure accurate referral pathways
Collaboration & Program Support
  • Work closely with Women’s Health, Outreach, Marketing, Informatics, and Finance teams as part of a cross-functional program
  • Provide feedback to program leadership on barriers, trends, and community needs
  • Participate in team meetings, training sessions, and quality improvement activities
  • Support stigma-reduction efforts by amplifying women-centered, affirming prevention messages (with materials developed by Marketing)
Documentation & Data
  • Accurately document encounters, outreach activities, and outcomes in PHNTX systems
  • Track required engagement and service metrics in collaboration with Informatics and program leadership
  • Maintain confidentiality and comply with all HIPAA and organizational privacy standards

What This Role Does NOT Include

  • Clinical decision-making, prescribing, or treatment
  • Managing or designing marketing campaigns
  • Owning partner strategy, calendars, or community-wide outreach plans
  • Grant reporting, budget management, or financial reconciliation
  • Supervising staff or volunteers

Qualifications

Required
  • Strong connection to and experience working with communities served by PHNTX
  • Experience in community outreach, patient navigation, health education, or social services
  • Excellent interpersonal and communication skills
  • Ability to discuss sensitive health topics with empathy and professionalism
  • Ability to work independently across clinic and community settings
  • Basic computer skills and ability to document activities electronically
 
Preferred
  • Knowledge of women’s health, HIV/STI prevention, or sexual health
  • Experience with PrEP navigation or prevention programs
  • Bilingual (English/Spanish)
  • Familiarity with trauma-informed and culturally responsive care models
