General Description:

This position provides behavioral intervention services, including testing and referrals, linkage to care, PrEP referrals, navigation and support services including medication adherence support and coordination with other behavioral intervention services and condom distribution.

Responsibilities

Specific Responsibilities of the Job:
  • Manages HIV testing site development and relationship building which includes site selection, obtaining necessary approvals, determining educational topics and scheduling.
  • Manages recruitment and enrollment of population of focus for participation in selected interventions.
  • Provides group or individual behavioral intervention services.
  • Conducts outreach to priority population.
  • Conducts evidence-based interventions.
  • Conducts individual risk screenings and assessments.
  • Provides Counseling Testing and Referral according to protocol – rapid and confirmatory testing, including finger stick or whole blood as required.
  • Assesses and makes appropriate referrals for interventions, testing, immunizations, case management, PrEP, and/or treatment.
  • Assists with adherence support/counseling.
  • Refers appropriate clients to other internal staff that can help them enroll in health insurance through the marketplace.
  • Assists in the coordination of behavioral intervention and treatment services provided by Prism Health North Texas and/or collaborating organizations.
  • Enters data into requisite databases.
  • Completes volunteer training through Windham school district at the Texas Department of Criminal Justice (TDCJ) to acquire access to all state prison units.
  • Ability to work some nights and weekends as needed.
  • Other duties as assigned by supervisor.

Required Knowledge, Skills and Abilities:
Job Requirements:
  • Proficiency in Excel, Word, Outlook and databases.
  • Must complete any trainings in preventive behavioral interventions in the first 3 months.
  • Must complete Navigator training within the first 3 months from start of program.
  • Ability to work effectively with diverse populations.
  • Dependable transportation and current Texas Driver’s License and active auto liability insurance.
Education and Experience:
  • High school diploma.
  • Bilingual preferred.
