This position provides outreach and education to HIV+ incarcerated and recently released offenders.
Responsibilities
Specific Responsibilities of the Job:
- Develops educational curriculum and presentation activities.
- Manages site development and relationship building, including site selection, obtaining approval, ensuring appropriate educational topics, recruitment of participants, and scheduling.
- Manages and conducts outreach to priority population within appropriate sites.
- Manages recruitment and enrollment of population of focus in the AIDS Drug Assistance Program (ADAP)
- Conducts brief risk screening.
- Conducts and documents needs/resource assessments of clients.
- Facilitates and provides prevention education related to HIV/AIDS and STDs.
- Assists with new client intakes, responding to walk-in clients and providing assistance with completing an ADAP application.
- Provides referral and care linkage services (medical care and ADAP).
- Addresses potential client barriers to medical care expeditiously and effectively.
- Follows all protocols, completes accurate and timely documentation of all client encounters into requisite databases as required and submits all necessary reports to supervisor on time.
- Completes volunteer training through Windham school district at the Texas Department of Criminal Justice (TDCJ) to acquire access to all state prison units.
- Other duties as assigned by supervisor.
Skills & Qualifications
Required Knowledge, Skills and Abilities:
Job Requirements:
- Proficiency in Excel, Word, Outlook and databases.
- Knowledge of medical/psychosocial resources and the medical and psychosocial complexities of HIV/AIDS.
- Knowledge of cultural variations across diverse populations.
- Ability to effectively communicate in verbal and written formats.
- Ability to collaborate with community service providers and establish effective working relationships with clients.
- Dependable transportation and current Texas Driver’s License and auto liability insurance.
Education and Experience:
- Bachelor’s degree in social work, psychology, public health or related fields.
- Must have 2 years of experience in social services preferably serving persons impacted by HIV/AIDS.
- Bilingual preferred.
