General Description:

The Free World Bound (FWB) HIV Navigator provides direct oversight and management of the FWB Texas linkage to care processes, lost to care, tracking viral suppression and reengagement to care. This position assumes responsibility for managing processes related to ensuring that FWB Texas clients at high risk for HIV get connected to the assigned Prism Health North Texas (PHNTX) medical provider. The position provides behavioral intervention services, including testing and referrals, linkage to care, navigation and support services, medication adherence support and coordination with other behavioral intervention services.

Responsibilities

Specific Responsibilities of the Job:
For the FWB Texas HIV Navigator:
  • Assesses clients' psychosocial needs and HIV medical care post-release.
  • Assists clients accessing psychosocial services, substance misuse and/or mental health disorder treatment services.
  • Links clients to HIV medical care, including making medical appointments and accompanying them to their medical appointments.
  • Ensures that clients are referred to and enrolled in Partner Services.
  • Assesses clients for medication adherence if on ART and refers to medication adherence services as needed.
  • Refers clients to HIV to prevention services as needed.
  • Works closely with clinical staff to ensure client compliance with all program criteria.
  • Assists all eligible clients with insurance assistance and/or pertinent patient assistance program.
  • Facilitates case conferences for staff engaged in prevention activities.
  • Conducts evidence-based interventions as required.
  • Conducts outreach to priority population.
  • Conducts individual risk screenings and assessments.
  • Conducts HIV, hepatitis C, and syphilis testing – rapid or by blood draw as required.
  • Assesses for and makes appropriate referrals to interventions, testing, immunizations, case management, and/or treatment.
  • Assists in the coordination of behavioral intervention and treatment services provided by Prism Health North Texas and/or collaborating organizations.
  • Enters data into requisite databases.
  • Manages recruitment and enrollment of population of focus for participation in selected interventions.
  • Manages site development and relationship building which includes site selection, obtaining necessary approvals, determining educational topics, and scheduling.
  • Submits reports and other documentation to program managers.
  • Provides supervision, training, and guidance to FWB Texas Linkage to Care Specialists.
  • Assists with data requests to external medical providers.
  • Completes volunteer training through Windham school district at the Texas Department of Criminal Justice (TDCJ) to acquire access to all state prison units.
  • Ensures that all lost to care clients are referred to DSHS Disease Intervention when contact efforts are unsuccessful.
  • Other duties as assigned by supervisor.

Skills & Qualifications

Required Knowledge, Skills and Abilities:
Job Requirements:
  • Proficiency in Excel, Word, Outlook and databases.
  • Training in and knowledge of preventive behavioral interventions.
  • Ability to work effectively with diverse populations.
  • Dependable transportation and current Texas Driver’s License and active auto liability insurance.
Education and Experience:
  • Bachelor’s degree in social work, psychology, public health or related fields. Significant experience in the relevant field may be considered in lieu of the degree.
  • Must have 2 years’ experience in behavioral health or social service and one year experience working with the priority population.
  • Bilingual preferred.
Special Working Conditions
  • Some evening and weekend hours. 
  • Some travel/driving required.
