General Description:
The Patient Navigator is responsible for overseeing and implementing the Patient First Program of “white glove” service to newly diagnosed or re-establishing HIV + patients seeking care at PHNTX by working closely with patients during the initial sixty (60) days of accessing care. Best practice in HIV management is to begin anti-viral medication within 72 hours and HIV viral suppression within 30 days of presenting for care. The primary goal for the Patient Navigator is to be in frequent contact (high touch) with the patient during the first 60 days, to ensure Rapid Start of HIV medication therapy, to provide the patient with the tools needed to self-manage their HIV, and to help the patient achieve HIV viral suppression within 60 days from initially presenting for care.

Responsibilities
Specific Responsibilities of the Job:
• Identify and outreach to all new HIV + patients and HIV + patients across all PHNTX clinics that are returning to care within a defined time frame as established by the program to facilitate getting patients into medical care and treatment quickly and efficiently.
• Provide regular follow up with all new or re-establishing HIV + patients as defined by the program as our Patient First Program (PFP), including at a minimum, ensuring patients are actively engaged in care and linked to appropriate resources to maintain adherence to their medical treatment regimen.
• Coordinate with pharmacy and case management to ensure patients initiate anti-retroviral medication within 3 days of the initial appointment, including assisting with completion of medication assistance program documentation.
• Ensure patients receive financial counseling and are screened for Ryan White benefits where applicable. Assists with document collection to support the qualifying for financial aid and other benefits that support managing a new HIV diagnosis.
• Communicate closely with provider, clinic, case management, and pharmacy staff to ensure coordination of services and messagingand ensure patient is supported in learning how to navigate Prism services.
• Maintain accurate and timely documentation in an electronic health record according to agency process and standards.
• Maintain client confidentiality and privacy and ensures all patient documents are stored in accordance with the Confidentiality and HIPAA security practices of the agency.
• Develop and implement the Patient First Program action plan in collaboration with the patient, caregiver, and provider, and PHNTX staff involved in the patient’s care.
• Assist in the process of referring patients to both internal and external service providers, and provide timely follows-up on referrals to ensure linkage to care, serving as a liaison to facilitate provision of service when needed.
• Coordinate a smooth transition from the Patient First Program to the clinical and / or case management team at the conclusion of the program.
• May require home and other off-site visits and / or assessments, as needed.
